My office accepts cash, checks, money orders, and debit/credit cards. I do not accept American Express.
All co-pays, deductible amounts, and private pay fees are due at the beginning of each appointment, unless other arrangements have been made in advance. My fee for an intake evaluation is $200.00, unless it is covered by your insurance. My fee for a regular 45 minute session is $150.00. Please be aware that if you are using your insurance, the 60-minute session may be scrutinized more closely than the standard 45 minute session, and more justification must be provided to the insurance company than simply “patient wants a 60-minute session.” Medical justification generally needs to address why the session went beyond the 45-minute standard (about 38 – 52 minutes).
If you miss an appointment or if you do not provide 24 hours advance notice of cancellation of an appointment (except in the case of emergency), you will be charged $75.00 for the missed appointment or late cancellation.
If you request any letters, forms, or any other paperwork to be completed, such as FMLA or disability forms, please be advised that there is a fee for paperwork. My fee is $150.00 per hour. FMLA paperwork generally requires a minimum of 30 minutes to complete, due to the need for supporting clinical documentation. Short-term disability often takes longer to complete, and may require additional assessments beyond my regular intake evaluation. The time required to make copies or prepare and send faxes, and any other administrative business (e.g. preparing releases of information or requests for records; phone calls to lawyers or other non-clinical calls) not directly related to the provision of clinical services, will also be assessed based on a rate of $150.00, with a minimum fee of $25.00.
I will not complete any FMLA, disability, other paperwork or letters of support unless I have met with you for at least 6-8 sessions. I will also not complete any FMLA or disability paperwork if I do not believe I can support it based on what you have presented at intake and during sessions.
As of September 1, 2013, I will be charging for telephone calls longer than 10 minutes. Please review the revised Office Financial Policy for details, and please be aware that I do not do therapy by telephone. My fees for telephone calls is $150.00 per hour; a 30-minute call will be $75.00. Please be advised that insurance companies have never reimbursed therapists for crisis phone calls; it is one of the reasons why clients are referred to crisis services. Although telehealth has become more acceptable and are now covered by some insurances, this generally does not include therapy by telephone.
